I've got a good one. Let me preface this with the fact that I don't socialize well in large groups of people. You can call me a wallflower in that regards. Anyway..

I started dating a girl I knew through mutual friends last summer after she asked me out one night. Things were going well after two dates. Then we went to my company summer picnic together, where I made the mistake of not introducing her to everyone I knew. That afternoon, I left for a 10 day business trip on the west coast. I called her a few days later and she confronted me on why I hadn't introduced her, if I was embarassed to be with her, etc. Apparently she had been stewing on that for the days before I called her, and I was 2000 miles away when she did let it all out. Anyways she was pissed and I couldn't make it up to her for another week, and that was the end of that. Looking back on it now, I probably should have sent her flowers or something, but hindsight is 20/20.. Luckily I have learned from that and am a little more keen on when a girl might have self-image issues that guys wouldn't pick up on!
