How do you use your name and password to call for map votes on the server

You don't use a name and password. Access is based on SteamID. You have to be a supporter or admin to get access.

I'm a supporter so could you direct me to a page where I can set that up

You just post your SteamID in the supporters forum or send it to an admin and they will get it set up.
